- finally submit the form
- Do not forget to attach necessary documents like birth certificate, passport size photograph, etc. along with it.
- Take out a copy of the form and keep it with you for future reference.

The main documents required at the time of admission are passport size photo of the child, passport size photo of the parent or guardian, photograph of the family, birth certificate of the child or Aadhaar card of the child, address proof, caste certificate ( If applicable), Aadhaar card of any one parent etc.
